Interim Healthcare is seeking a Home Health RN - Baylor to work full-time with our clients and our home health team in Dorchester, Charleston, and Berkeley Counties.

Responsibilities
- Perform assessments and procedures requiring specialized nursing knowledge and judgment
- Assess and review employee skills to match client needs
- Evaluate the quality of care provided during home visits and review clinical notes
- Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s)
- Obtain and execute physicians' plans of treatment and orders
- Counsel and educate patients, clients, and families on healthcare needs
- Complete timely and accurate clinical notes, including patient progress
- Communicate effectively with the interdisciplinary healthcare team to coordinate care and plan for discharge
- Supervise paraprofessional and licensed professional team members
- Secure equipment and supplies and coordinate their placement in the home
- Refer patients to other services as needed
- Complete additional assignments as requested
- Participate in the Quality Assessment and Performance Improvement (QAPI) program
- Attend in-services or training as requested
- Participate in the Office’s Emergency Preparedness program
- Access personal health information (PHI) necessary to fulfill job duties
Requirements
- One year of home health nursing experience required; two years preferred
- Demonstrated understanding of computers and workplace software
- Compassionate, able to function well under stress, and use clinical judgment
- Ability to handle multiple changes based on patient needs and scheduling
- Occasional travel within the geographic area serviced by the office
- Frequent lifting, repositioning, or transferring of patients, and pushing/pulling up to 50 lbs
- Current CPR certification
- Valid driver’s license and auto insurance
- Graduate of an accredited school of nursing with a current RN license
