This position is a multi-functional role that serves as a main point person for the bariatric patient in the office setting. The role involves providing comprehensive bariatric services, building lasting relationships with patients, and serving as an advocate for successful outcomes.

Minimum Job Requirements
- High School Degree or GED
- Licensed Practical Nurse
- 2-3 years nursing experience
Job Responsibilities
- Greets patients and presents, reviews, and answers all questions during the initial overview of the program and the program binder including all pre-surgical requirements after determining the length of the program based on the patient's insurance, as well as scheduling patients program appointments including class and Behavioral Health at the initial consult.
- Facilitates and provides education for the pre-surgery bariatric patient class, triages all patient needs, requests, and questions for pre-surgical, acute post-surgical, and the long-term post-surgical patient, provides chart review and pre-op clearance coordination.
- Carefully coordinates the care of the high-risk bariatric surgery patient for full optimization in preparation for surgery and places post-operative follow-up calls.
- Answers the designated bariatric telephone line, as well as the main line when appropriate. Triage phone calls, answer basic questions about the bariatric program, maintain and assemble seminar binders and patient charts/documents.
- Maintains the patient spreadsheet for designated region, keeping it up to date with all patient progress daily.
- Works in tandem with the new Nurse Practitioner supporting patient pre-surgery visit outlining expectations and essential post op compliance education and guidelines. Supports APRN with post surgical hospital discharge planning.
