Key Responsibilities

• Providing patient care in home settings with empathy and clinical expertise
• Utilizing electronic documentation via tablets in patient homes
• Collaborating with a multidisciplinary team to achieve positive clinical outcomes

Qualifications

• Active, unencumbered LPN/LVN license in the practice state
• Current CPR certification (training available post-hire if needed)
• TB skin test within last 12 months
• Reliable transportation
• Experience preferred but not required
