Charlotte, North Carolina – Avant Pharmacy & Wellness Center (formerly Rx Clinic Pharmacy) offers a Population Health and Practice Development Fellowship focused on expanding pharmacist-led collaborative clinical services.
About the role
This is the first year we are offering a community-focused fellowship position. The fellow will serve as the primary clinical pharmacist at partnered clinics, driving patient outcomes through community clinical programs and gaining hands-on experience in identifying drug therapy problems. The role includes a staffing component to deepen understanding of patient care at the point of service.
As a pharmacist fellow, you will work alongside our Director of Health and Wellness to:
- Implement a dementia care model supporting patient independence at home, collaborating with home care agencies and assisted living facilities.
- Lead comprehensive weight management programs incorporating lifestyle, behavioral, and medical approaches.
- Assist with medical infusions and IV hydration by coordinating patient care, monitoring medications, and ensuring adherence.
- Support hypertension and diabetes therapy management.
- Contribute to the expansion of pharmacy services and foster interprofessional collaborations to enhance patient care.
Primary clinical services
In-clinic services
- Annual wellness visits (AWVs): Annual comprehensive visit for Medicare patients.
- Chronic care management (CCM): Management for Medicare patients with two or more chronic conditions.
- Remote physiologic monitoring (RPM): Management of digitally collected physiologic data.
- Behavioral health integration (BHI): Management for Medicare patients with one or more behavioral health conditions.
Community clinical services
- Medication Therapy Management (MTMs): Comprehensive medication reviews and targeted intervention programs.
- Private payer contracts: Targeted programs to help patients achieve health goals through pharmacist management.
- Wellness program initiatives: Incorporating lifestyle, behavioral, and medical approaches to support patient success.
- Community pharmacy staffing: Identify key drug therapy problems through patient profile management.

About the team
- Brianna Johnson, PharmD – Site coordinator. Specializes in geriatric and collaborative care with independent physicians. Graduate of Wingate University School of Pharmacy. Experienced in initiating and coordinating collaborative programs, presenting at pharmacy organizations, and co-facilitating fellowships.
- Amy Kern, PharmD – Site preceptor for community clinical position. Graduate of North Dakota State University. Over 20 years of experience in chain and independent pharmacies, including compounding, care coordination, COVID-19 vaccination clinics, 340B program management, and HIV treatment processes.
- Jack Novansy, PharmD, MBA – Site preceptor for community clinical position. Director of Health and Wellness at Avant Pharmacy & Wellness Center. Graduate of East Tennessee State University. Combines expertise in exercise science and pharmaceuticals to optimize health and promote active, healthy lifestyles.
