About the role

Fairview Skilled Rehabilitation Center & Nursing Home is a high-quality post-acute and long-term care facility dedicated to providing excellent medical care in a respectful, collaborative environment. The interdisciplinary team works closely together to support residents through recovery, rehabilitation, and long-term care, maintaining strong communication with families and hospital partners.

Key Responsibilities

  • Provide comprehensive medical care to residents in skilled rehab and long-term care
  • Conduct initial evaluations, routine follow-ups, and acute change-in-condition assessments
  • Manage chronic and complex medical conditions
  • Oversee admissions, readmissions, and discharge planning
  • Review and sign orders, progress notes, and care plans
  • Collaborate with nursing leadership, rehabilitation therapists, social work, and administration
  • Communicate effectively with residents and families
  • Supervise and mentor limited permit physicians, nurse practitioners, or physician assistants (as applicable)
  • Ensure compliance with state, federal, and facility regulations
  • Support quality improvement initiatives and best clinical practices
  • Coordinate with hospitals, specialists, and outside providers as needed

Qualifications & Requirements

  • MD or DO from an accredited medical school
  • Active New York State Medical License | Board Certified or Board Eligible in: Internal Medicine, Family Medicine, Geriatrics (preferred, not required)
  • Strong clinical judgment and decision-making skills
  • Excellent communication and documentation abilities
  • Ability to work collaboratively within an interdisciplinary team
  • Comfort working in a skilled nursing and rehabilitation environment
  • Preferred experience in: Skilled nursing facilities, Post-acute or rehabilitation settings, Long-term care, Familiarity with EMR systems, Experience supervising mid-level providers or limited permit physicians
