About the role
Fairview Skilled Rehabilitation Center & Nursing Home is a high-quality post-acute and long-term care facility dedicated to providing excellent medical care in a respectful, collaborative environment. The interdisciplinary team works closely together to support residents through recovery, rehabilitation, and long-term care, maintaining strong communication with families and hospital partners.
Key Responsibilities
- Provide comprehensive medical care to residents in skilled rehab and long-term care
- Conduct initial evaluations, routine follow-ups, and acute change-in-condition assessments
- Manage chronic and complex medical conditions
- Oversee admissions, readmissions, and discharge planning
- Review and sign orders, progress notes, and care plans
- Collaborate with nursing leadership, rehabilitation therapists, social work, and administration
- Communicate effectively with residents and families
- Supervise and mentor limited permit physicians, nurse practitioners, or physician assistants (as applicable)
- Ensure compliance with state, federal, and facility regulations
- Support quality improvement initiatives and best clinical practices
- Coordinate with hospitals, specialists, and outside providers as needed
Qualifications & Requirements
- MD or DO from an accredited medical school
- Active New York State Medical License | Board Certified or Board Eligible in: Internal Medicine, Family Medicine, Geriatrics (preferred, not required)
- Strong clinical judgment and decision-making skills
- Excellent communication and documentation abilities
- Ability to work collaboratively within an interdisciplinary team
- Comfort working in a skilled nursing and rehabilitation environment
- Preferred experience in: Skilled nursing facilities, Post-acute or rehabilitation settings, Long-term care, Familiarity with EMR systems, Experience supervising mid-level providers or limited permit physicians
