Associate Medical Director
About the role

Responsibilities
- Perform in-person comprehensive assessments and coordinate with the IDT to develop a care plan for each participant.
- Oversee all care of participants in nursing homes and hospitals, including admits and discharges for acute, short-term, and long-term placement, and the use of medical specialists to ensure effective care coordination.
- Provide assessment, planning, primary care, and continuous medical coverage to participants.
- Collaborate with community-based physicians and IDT in discharge planning from acute or long-term settings.
- Actively participate in IDT, communicating participant changes, collaborating on care planning decisions, and coordinating 24-hour care delivery.
- Maintain care of participants in nursing homes, making regular visits as dictated by participant need.
- Take part in rotating night and weekend call.
Requirements
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
- Master’s degree/doctorate degree.
- Board Certification – Internal Medicine or Family Practice.
- Board Certification – Geriatrics, preferred.
- Active California Physician License without restrictions.
- Active state CDS Certification and DEA.
- Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants.
- Obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Pay
$303,534—$370,011 USD

Qualifications
Experience in geriatric care, familiarity with PACE programs, and a passion for value-based care are highly valued.
Schedule
Rotating night and weekend call may be required.
Pay
Compensation offering includes base salary plus bonus.
