Associate Discharge Planner
About the role
Under the general supervision of the Manager of Clinical Resource Management, the Associate Discharge Planner supports the department in discharge planning implementation and administrative tasks. The Associate Discharge Planner implements the delegated discharge plan under the supervision of the care manager, registered nurse, or provider.

Requirements
- Three (2) years of clinical or administrative experience in the healthcare industry
Qualifications
- Bachelor's Degree in Social Work (preferred)

Pay
The pay range for this position is $28.11 - $43.00 per hour. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
Schedule
Full time, Day Shift, 40 working hours weekly.
