What You’ll Be Doing
- Provide direct supervision for Care Transitions Specialists (CTSs); recruit, train, and develop clinical refreshers to ensure high-quality client care. Serve as a substitute CTS when needed.
- Act as program liaison and trainer for medication adherence initiatives—mitigating barriers, supporting 90-day conversions, and assisting high-risk non-adherent members.
- Partner with the Palliative Care Supportive Services team to oversee the New York Center for Dignity and Support, ensuring program quality and monthly target achievement.
- Oversee the Health Home program, drive program growth, and foster integration between Health Home and Care Transitions workflows.
- Partner & Business Development: Strengthen relationships with hospital systems and health plans; lead partner meetings, prepare case studies/presentations, and support business development efforts.
- Manage program evaluation efforts, complete aggregate monthly reports, and use data analysis to optimize program strategy and operations.
What You’ll Need
- Education: International Medical Graduate (IMG) or equivalent degree required.
- Language Skills: Bilingual English/Spanish required (ability to read, write, speak, and understand both languages).
- Leadership Experience: Minimum of 3 years of managerial and leadership experience with a proven track record of staff training and skill transfer.
- Technical & Process Skills: Advanced proficiency in Microsoft Office and Google Workspace; experience with Electronic Health Records (EHRs).
- Experience with process improvement, workflow automation, Python, AI tools, or data analysis techniques is highly desirable.
- Sector Expertise: Background in hospital discharge planning or healthcare settings preferred.
- Travel & Physical Requirements: Ability to travel to clients' homes via mass transit and negotiate steps/physical barriers in residences.
