About the role

The Associate Center Medical Director provides day-to-day medical operations support at a specified center under the direction of the Sr. Center Medical Director or Center Medical Director. This position includes direct patient care for InnovAge participants across various settings, such as the center’s clinic, participants’ homes, nursing homes, assisted living facilities, and personal care boarding homes. While direct hospital inpatient care is not required, close coordination with hospital-based providers is essential.

Responsibilities

  • Participate within the interdisciplinary team to formulate Plans of Care for InnovAge PACE participants and support their care in other interdisciplinary settings.
  • Provide medical care to InnovAge PACE participants, including evaluation, diagnosis, and management of medical conditions.
  • Conduct comprehensive medical histories, physical examinations, order and interpret diagnostic tests, formulate assessments and treatment plans, write prescriptions, and manage ongoing follow-up.
  • Document medical care in participant records in compliance with InnovAge PACE documentation policies.
  • Admit and manage participants in Nursing Facilities under contract with InnovAge and fulfill nursing facility-required medical staff duties.
  • Supervise and manage assigned direct reports, providing leadership, motivation, and direction to employees.
  • Coordinate and conduct regular primary care meetings.
  • Support strategic goals and local primary care initiatives of InnovAge.
  • Assist the SCMD or CMD with quality initiatives, inpatient and emergency department utilization, outpatient specialist utilization, regulatory compliance, pharmacy utilization, and short-stay nursing home utilization.
  • Present input, initiate, or facilitate new medical programs and processes in support of InnovAge PACE’s strategic goals.
  • Assist with department budget preparation, quality initiatives, and other assigned duties.
  • Support primary care capability through hiring, training, planning, assigning work, appraising performance, and addressing employee relations issues.
  • Supervise and provide after-hours medical coverage to ensure physician availability 24/7, 365 days per year.
  • Ensure annual employee performance reviews are conducted timely and effectively.
  • Maintain department morale, teamwork, and a positive employment culture.

Travel Requirements

  • Travel to participant and potential participant homes and other off-site locations.
  • Occasional overnight out-of-state travel may be required.

Qualifications

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below represent the necessary knowledge, skill, and/or ability.

  • Doctor of Medicine or Osteopathy degree.
  • Board Certified in a primary care specialty (Board Eligible considered but must pass boards within 12 months of employment).
  • Current active Medical License for the state in which practicing.
  • DEA License with full prescriptive authority.
  • Adequate malpractice coverage.
  • Not on the Preclusion List.
  • A minimum of one year working with the frail or elderly.

Preferred Qualifications

  • Postgraduate training in a primary care specialty, such as internal or family medicine.
  • Completion of post-residency training in geriatric medicine.
  • Bilingual.

Pay

Posted pay range: $300,000 - $340,000.
