About the Opportunity
The Associate Care Advocate role focuses on supporting patients by identifying health-related challenges and connecting them to appropriate community resources. This entry-level position is critical in facilitating patient engagement and access to services, contributing to the overall effectiveness of community care programs. The role offers a clear progression path to more senior positions within the care delivery team.
Responsibilities
- Conduct phone screenings using scripted questions to understand patient challenges
- Build trust and create a comfortable environment for honest patient communication
- Connect patients with health, mental health, and social service resources
- Accurately document patient information and consent in the Electronic Health Record system
- Maintain compliance with federal privacy regulations and billing standards
- Manage daily scheduling, send appointment reminders, and schedule follow-up calls
- Meet weekly performance goals
Requirements
- High school diploma or GED required; degree in Human Services, Public Health, or related field preferred
- Bilingual in English and Spanish or another language strongly preferred
- Community Health Worker certificate preferred or willingness to obtain certification through training
- Comfortable engaging with diverse populations over the phone
- Basic technical proficiency with websites and data entry applications
- Ability to work independently and manage tasks with minimal supervision
- Successful completion of onboarding training including call practice and documentation exercises
- Ability to pass background check and drug test
- Physical ability to sit, stand, walk as needed, and use computer equipment
- Access to reliable internet and a quiet, private workspace for calls
Pay Range and Compensation Package
$18 to $20 per hour
