About the role

The Health Services Director oversees the health services department, ensuring resident care needs are met by coordinating care, managing systems, and supervising the health services team. This role promotes a positive physical and psychosocial environment for residents.

Responsibilities

  • Creates a resident-centered approach to planning and delivering services, supporting residents’ rights to participate in decision-making and treating residents with dignity and respect.
  • Performs assessments on health, functional, and psychosocial status of residents and participates in the development of individualized service plans.
  • Proactively manages care and services for each resident, maximizing their opportunity to remain in the assisted living environment.
  • Problem-solves as part of the management team to case manage and meet resident needs.
  • Assesses or evaluates residents with reported changes of condition.
  • Provides team member direction, service planning, and coordination of care to meet residents' changing needs.
  • Collaborates with the management team on the move-in of prospective residents, considering their needs, community acuity, and staffing levels.
  • Provides in-person pre-move-in assessments when necessary.
  • Ensures resident care needs are met by coordinating services with physicians, discharge planners, nurses, home health agency staff, case managers, hospice, and state agency personnel.
  • Oversees the arrangement of ancillary services such as pharmacy, podiatry, lab, and x-ray as needed.
  • Communicates regularly with residents’ families regarding health care needs and provides education and support.
  • Provides supervision and training to health services team members, including personal care and medication management.
  • Manages the delegation of nursing tasks as allowed under applicable state nursing laws and practice acts.
  • Partners in the investigation and root cause analysis of incidents and collaborates on the development and implementation of appropriate interventions.
  • Oversees the documentation and communication of resident care and services, ensuring records are completed timely and maintained according to policies and regulations.
  • Assists in the management of Infection Control, including staff training, incident investigation, and tracking infection data.
  • Manages and oversees health services systems, maintaining up-to-date knowledge of rules, regulations, and policies pertaining to resident care.
  • Stays informed of industry and health care trends, applying new knowledge to meet residents' changing needs.
  • Provides on-call support and phone triage for team members’ questions regarding resident concerns.
  • Participates in leadership team meetings, including daily stand-ups, Systems Monitoring and Resident Tracking (SMART), and Quality Assurance Performance Improvement (QAPI).
  • Acts as an ambassador of Prestige by representing the community positively to outside agencies and participating in community events.
  • Performs other duties as assigned.

Qualifications

  • Current, unencumbered LPN/LVN or RN license in the state of practice (RN strongly preferred).
  • Experience in geriatric nursing, medication administration, case management, training/staff development, budgets, and team management preferred.

Education

LPN/LVN, RN, Associate Degree in Nursing, or Bachelor of Science in Nursing.
