Assisted Living Nursing Director - RN

About the Role

The Health Services Director role is truly unique and offers a Monday-Friday daytime schedule with significant flexibility. You will lead the health services department, overseeing the entire care program, including coaching and teaching unlicensed staff such as Personal Care Attendants, Medication Technicians, and Resident Care Coordinators.

Responsibilities

• Create a resident-centered approach to planning and delivering services, supporting residents’ rights to participate in decision-making and treating them with dignity and respect.
• Perform assessments on health, functional, and psychosocial status of residents and participate in developing individualized service plans.
• Proactively manage care and services for each resident to maximize their opportunity to remain in the assisted living environment.
• Problem-solve as part of the management team to case manage and meet residents' needs.
• Assess and evaluate residents with reported changes of condition.
• Provide team member direction, service planning, and coordination of care to meet residents' changing needs.
• Collaborate with the management team on the move-in of prospective residents, considering their needs, community acuity, and staffing levels.
• Conduct in-person pre-move-in assessments if necessary.
• Ensure resident care needs are met by coordinating services with physicians, discharge planners, nurses, home health agencies, case managers, hospice, and state agency personnel.
• Oversee the arrangement of ancillary services such as pharmacy, podiatry, lab, and x-ray as needed.
• Communicate regularly with residents’ families regarding health care needs and provide education and support.
• Provide supervision and training to health services team members, including personal care and medication management.
• Manage the delegation of nursing tasks as allowed under applicable state nursing laws and practice acts.

Requirements

• Current, unencumbered LPN/LVN or RN license (RN strongly preferred).
• Experience in geriatric nursing, medication administration, case management, training/staff development, budgets, and team management preferred.
• Associates Degree or Bachelor of Science Degree in Nursing.
