Assisted Living Case Manager

About the role

We are currently seeking a dedicated and passionate Case Manager with experience in senior living to join our team.

Responsibilities

• Orient and assist each resident as they adjust to life in the community.
• Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to the community, including addressing their social, emotional, and psychological needs.
• Assist each resident in maintaining family and community connections while encouraging the development of new relationships.
• Complete quality management reviews to ensure the department meets state regulations and Peregrine Senior Living policies, and follow up as needed.
• Collaborate with the care department and other team members to promote and protect resident rights and support each resident's psychological well-being.

Requirements

• Bachelor's degree with major coursework in Human Services.
• At least one year of full-time work experience in a related field.
• Case management experience required.
• Memory Care or Assisted Living experience required.
• Compassion for and a genuine desire to work with older adults.

Pay

$63,000–$66,000 annually, based on experience
