Responsibilities
- Complete the questionnaire thoroughly and submit all requested information.
- Upload relevant certificates and/or screenshots verifying business status.
- Provide details on any judgments, arbitrations, liens, insurance claims, or lawsuits within the past 5 years.
- Answer questions regarding criminal background, including convictions and charges within the past 5 and 10 years.
- Detail insurance coverage limits and willingness to purchase additional coverage.
- Describe your Health & Safety Program, including written policies, site-specific plans, inspections, investigations, and training.
- Indicate compliance with OSHA standards and maintenance of OSHA logs.
