Assistant Clinical Manager (RN) - Home Health and Hospice

About the role

The Assistant Clinical Manager (ACM) at Home Health and Hospice care plays a crucial role in supporting compassionate, patient-centered care teams. This role guides and empowers these teams to deliver safe, compliant, high-quality care in the homes of those they serve.

Responsibilities

• Provide clinical oversight during assigned coverage (including weekends/holidays and off-hours as needed)
• Schedule and supervise designated staff
• Coordinate care delivery and handoffs
• Serve as a point of escalation for clinical and operational concerns
• Partner closely with interdisciplinary and administrative teams to ensure continuous care seven days a week and adherence to organizational policies and applicable regulations

Requirements

• Associate or Bachelor’s degree in Nursing from an accredited program
• Current, unrestricted RN license in the state of practice and BLS certification
• Hospice and Palliative Nursing certification (preferred) or willingness to obtain within 1 year
• Three years’ experience as a Registered Nurse in Home Health and/or Hospice/Palliative care
• Two years progressive leadership/supervisory experience

Qualifications

• Demonstrate and apply working knowledge of hospice/home health regulations, standards of practice, and clinical workflows to promote compliance, efficiency, and high-quality outcomes
• Apply clinical judgement to balance patient needs, regulatory requirements and organizational policies in care planning and delivery
• Maintain productivity and quality of work while managing frequent interruptions and multiple concurrent responsibilities
• Demonstrate effective written and verbal communication skills when interacting with staff, leadership, patients, and interdisciplinary partners

Sunday – Wednesday (ACM position one) or Wednesday – Saturday (ACM position two)

Typical Hours: 7:30 AM – 4:30 PM
