Assessor Reviewer I

About the role

The Assessor Reviewer I is responsible for reviewing and processing PASRR (Pre-Admission Screening Resident Reviews) Level 1 reviews of vulnerable populations and individuals entering or living in nursing facilities. Persons being assessed reside in the community, hospitals, or facilities. The incumbent may also complete review decisions regarding additional assessments needed for the person(s) while in a nursing facility. This position may also serve as a mentor or preceptor for new staff, assisting in the formal training of assessors, completing quality assurance competency reviews, and may be involved in process improvement initiatives.

Responsibilities

• Performs Pre-admission and Resident Review (PASRR) Level 1 screenings to determine if a PASRR Level 2 is needed for individuals with a known or suspected major mental illness and/or Intellectual Developmental disability entering or living in nursing facilities.
• Collaborates with health care providers and ancillary service providers to create or validate the appropriate outcome.
• Documents Level 1 screening outcome in Korak’s web-based portal.
• Provides ongoing education.
• Attends quality assurance competency training and quality improvement efforts as assigned.
• May support other assessors in being trained as needed.

Requirements

• A four-year degree in health care or a related field (Psychology or social work degree preferred).
• An understanding of medical terminology to include mental illness diagnoses awareness and I/DD experience.
• Strong communication skills and experience reviewing and interpreting medical documentation.
• Experience in human services and working with individuals who have disabilities is strongly preferred.
• QMHP or related certifications preferred.
