About the role
Provides hospice care services to patients and families in their homes. Works closely with interdisciplinary teams to coordinate care plans.
Responsibilities
- Administers medications and treatments as prescribed
- Assesses patient needs and documents progress
- Collaborates with healthcare providers to develop and implement care plans
- Maintains accurate records and reports
- Communicates with patients, families, and other healthcare professionals
Requirements
- Bachelor's degree in a healthcare-related field
- Current CPR certification
- Experience in hospice or home health care preferred
Qualifications
- Strong communication skills
- Ability to work independently and as part of a team
- Flexibility to work evenings and weekends

- Knowledge of hospice care principles and practices
