APRN: School Based Clinics

About the role

HealthFirst Bluegrass is seeking a compassionate, mission-driven APRN to provide high-quality primary care services in our School-Based Health Clinics. This position focuses on caring for children and adolescents while also supporting patients of all ages within our community health system. As part of our team, you'll help remove barriers to care, improve student health outcomes, and support families through accessible, patient-centered healthcare.

Responsibilities

• Provide preventive, acute, and chronic care services
• Perform assessments, diagnoses, and treatment planning
• Educate patients and families on wellness and self-care
• Collaborate with school personnel, community partners, and healthcare teams
• Support HealthFirst Bluegrass's mission of expanding access to quality healthcare for all

Qualifications

• Current Kentucky APRN license
• National Nurse Practitioner certification
• CPR certification
• Pediatric, family practice, community health, or school-based experience preferred
• Passion for working with children, families, and underserved populations
