Specific Responsibilities
- Conducts comprehensive mental health assessments of patients with chronic mental health conditions.
- Establishes psychiatric diagnoses and treatment plans.
- Collaborates with and provides expertise to the multidisciplinary team regarding patient’s treatment.
- Writes specific orders within the limits of the practice agreement/scope of practice.
- Provides appropriate direction to the team regarding patient care issues.
- Communicates effectively with community partners.
- Participates in a team model of care delivery.
- Participates in quality assessment and improvement activities.
- Is responsible for timely and accurate written documentation or dictation of patient care encounters/procedures.
Patient Care
- Provides care that is patient-centered, compassionate, appropriate, and effective for the promotion of health, prevention of illness, and treatment of disease throughout the lifespan.
- Facilitates consistent, coordinated care and clear communication among all team members.
- Incorporates the patient and family in care planning and treatment across settings.
- Serves as an advocate for patients and families in navigating the healthcare system.

- Follows ethical principles regarding patient confidentiality, informed consent, and unanticipated adverse outcomes.

Must-Have Requirements
- Master's or Doctorate degree from an accredited nursing or PA program.
- Active Utah State license to practice within APRN/PA scope.
- Active DEA license or ability to apply for such license.
- Not excluded from participating in government programs (OIG exclusion list).
- CPR certification and advanced life support certifications, if relevant to the practice area.
- (ACLS, PALS, NRP)
- Ability and/or willingness to become Medication Assisted Treatment waived.
- Ability to care for acutely and chronically ill patients.
- Excellent verbal and written communication skills.
- Professional demeanor.
- Ability to work as a member of a multidisciplinary team.
- Ability to prioritize tasks and complete required documentation with timeliness and accuracy.
Pre-Employment Requirements
