About the role

We are partnering with an established healthcare organization seeking an experienced APRN / NP to provide comprehensive care to geriatric patients in skilled nursing and post-acute settings.

Responsibilities

• Provide comprehensive medical management for residents
• Evaluate new admissions and manage ongoing patient care
• Diagnose and treat acute and chronic conditions
• Medication management and treatment-plan development
• Collaborate closely with physicians, nursing staff, therapists and other healthcare professionals

Requirements

• Current APRN / NP license in the state of practice
• Board Certification as an NP
• Active DEA registration, as applicable

Preferred qualifications

• Geriatric, Skilled Nursing, Long-Term Care, Post-Acute, Internal Medicine, Family Medicine or related experience
• Strong assessment, communication and clinical decision-making skills
• Comfortable managing medically complex geriatric patients
• Passion for providing quality, patient-centered care
