About the role
This is a unique opportunity to apply your deep and broad knowledge to Senscio Systems' cutting-edge Continuous Care Program. We are seeking an experienced Nurse Practitioner who is passionate about supporting patients with complex medical challenges.
Responsibilities
- Leverage the virtual care platform to support patient self-management through close engagement with participating Physicians, RNs, Behavioral Health and Community Health workers and other members of the multi-disciplinary care team.
- Provide actionable situational awareness to participating Physicians, RNs, Behavioral Health and Community Health workers and other members of the multi-disciplinary care team.
- Integrate physical and behavioral health needs with a broad scope of social determinants of health needs, providing a truly holistic, patient-centered plan of self-management support.
- Utilize AI-based technology to efficiently administer care, spending less time documenting and more time directing the care of your patient.
Requirements
- Board Certified Nurse Practitioner with experience in Primary Care, Family Practice and/or Geriatrics.
- Minimum of master’s degree in nursing.
- APRN license in good standing in Massachusetts with additional licensure in Florida, Maine, New Hampshire, Rhode Island, Pennsylvania, North Carolina, and Connecticut a strong plus.
- Minimum of 10+ years of experience managing complex patients.
- Desired: Systems mindset, translating assessments into coordinated tasks, interest and willingness to serve in multiple roles, including direct patient care, strong back-up and support to Physicians, teaming with and leadership of RN’s and other members of a broad based clinical and non-clinical team, strong clinical reasoning + decisiveness under uncertainty, comfort in a highly dynamic environment, willingness to obtain licensure in all states/regions where Senscio has operations, experience/interest in telehealth service delivery, and passion for providing Chronic Care Management.
