Purpose

The Nurse Practitioner at Nest Health improves health outcomes for underserved families through culturally congruent, longitudinal care that integrates physical, behavioral, and social health. As part of an interdisciplinary team, you’ll advance Nest’s mission to make comprehensive, value-based care radically accessible to America’s highest-risk populations.

Responsibilities

  • Manage a panel of families as the primary care provider, delivering longitudinal primary and preventive care, care management, and coordination in community facilities, homes, or via virtual modalities (video, phone, SMS).
  • Practice with the support of an interdisciplinary team, including MDs, LCSWs, RNs, psychiatrists, and medical assistants.
  • Utilize Nest Health’s electronic health record and care management tools to document member interactions, track tasks, and communicate with your team, members, and community resources.
  • Attend daily team huddles and weekly case conferences to advise on physical health needs and establish health goals.
  • Propose treatments for chronic and infectious diseases, prescribe and reconcile medications, and educate patients about their conditions.
  • Order, administer, obtain, and record diagnostic tests.
  • Provide advocacy, navigation support, and education to families on health and social care services, including eligibility and availability.
  • Participate in outreach efforts to explain program details to potential families.

Requirements

  • Completion of a Physician Assistant or Nurse Practitioner accredited program and an active, unrestricted license to practice in Arizona.
  • At least 2 years of experience providing clinical services to pediatric (including newborns) and adult patients with chronic medical and behavioral health conditions, with an interest in serving underserved, vulnerable populations.
  • Demonstrated proficiency or willingness to train in clinical nursing skills such as blood drawing (venipuncture & phlebotomy), patient assessments, and care plan reinforcement for conditions like diabetes, respiratory concerns, hypertension, CHF, and depression.
  • Proven skills, knowledge, and judgment for independent clinical decision-making and problem-solving.
  • Availability to work full-time, five days a week, and participate in a rotating call schedule that includes occasional evenings and weekends.
  • Current CPR certification (BLS).
  • Experience leading an interdisciplinary care team is preferred.
  • Startup experience is a plus but not required.
  • Access to reliable transportation and willingness to comply with Nest’s Motor Vehicle Safety Policy.
  • Willingness to educate and encourage vaccination against COVID-19.

Travel & Physical Requirements

  • Travel between work locations in Bullhead City, AZ, including current and future service areas, may be required in emergent situations.
  • Must have a driver’s license, a car (mileage reimbursed), auto insurance, and the ability to drive to the homes of Nest families while carrying portable healthcare supplies.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Ability to lift up to 25 pounds at times (e.g., medical equipment).
