About the role
The Nurse Practitioner or Physician Assistant at Nest Health is designed to improve health outcomes for underserved families through culturally congruent, longitudinal care that integrates physical, behavioral, and social health. Operating within an interdisciplinary team, the NP/PA plays a vital role in advancing Nest’s mission to make comprehensive, value‑based care radically accessible to America’s highest‑risk populations. The NP or PA serves as the primary care provider, delivering whole‑family primary, acute, chronic, and pediatric care to adult and pediatric family members in the home setting, while also acting as an educator and collaborating closely with physicians, behavioral health counselors, registered dieticians, and medical assistants upskilled to support social services navigation.
