APP - Certified Nurse Practitioner | Hospice

About the role

The Certified Nurse Practitioner will provide holistic, compassionate care to clients and families, collaborating with physicians, staff RNs, and other healthcare professionals.

Responsibilities

• Collaborates with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel.
• Identifies overall quality improvement activities through assessing and planning health care.
• Reviews and interprets medical, surgical, and social histories of clients.
• Performs appropriate screening procedures for clients.
• Interprets laboratory and other diagnostic data and recommends appropriate treatments or actions for follow-up.
• Sets up and performs various treatment modalities including, but not limited to: medications, injections, wellness counseling, suturing, and ancillary testing.
• Aids hospice facility employees in maximizing client quality of life.
• Evaluates clients who have acute changes in condition, and collaborates with the primary care provider and employees to implement appropriate interventions.
• Supports open access for clients to agency services through collaboration with the interdisciplinary team.
• Provides medical expertise on pain, symptom management, and the evaluation of terminal prognosis to clients, family members, and staff.
• Ensures compliance with OSHA, State Board of Nursing, and HIPAA regulations.
• Identifies unmet needs and gaps in care, and initiates appropriate referrals as indicated.
• Creates and revises the client's plan of care.
• Communicates and collaborates with Facility leadership to achieve client care needs and goals.
• Participates in different roles or interdisciplinary teams as assigned.
• Aids on special projects as assigned.
• Attends client meetings as requested.

Requirements

• Completion of a master’s, postmaster’s, or doctorate from an nurse practitioner program accredited by the Commission on the Collegiate of Nursing Education or National League for Nursing Accrediting Commission.
• Licensing prior to August 1, 1995 in ND, master's degree in nursing is preferred.
• Minimum two years clinical experience as a Nurse Practitioner in geriatric, adult, or family practice setting.
• Experience with palliative care or hospice preferred.
• Demonstrated current competence and provision of care, treatment, or services for an adequate volume of patients in the past twelve months, or completion of master’s/post-master’s degree program in the past twelve months.
• Experience must correlate to the privileges requested.
• Current licensure by the applicable state board of nursing for advanced practice.
• Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances.
• Current certification by the American Nurses Credentialing Center or an equivalent nationally recognized body.
• Obtains and subsequently maintains required department specific competencies and certifications.
