About the role
At Anthem Hospice, we believe every patient deserves comfort, dignity, and compassionate care during life's most important journey. We are seeking an experienced and compassionate Registered Nurse (RN) to join our growing team as a Hospice RN Case Manager.
Responsibilities
- Complete comprehensive patient assessments and ongoing evaluations
- Develop, implement, and revise individualized plans of care
- Manage pain and symptoms to maximize patient comfort and quality of life
- Provide skilled nursing services in accordance with physician orders and hospice regulations
- Educate patients, caregivers, and families regarding disease progression, medications, and care planning
- Cookordinate care with the interdisciplinary hospice team
- Serve as a clinical advocate for patients and families
- Provide emotional support and guidance during challenging times
- Maintain accurate and timely documentation
- Participate in interdisciplinary team meetings and care conferences
- Ensure compliance with agency policies, Medicare regulations, and hospice standards of practice
Requirements
- Current California Registered Nurse (RN) license in good standing
- Minimum of one year nursing experience
- Valid driver's license, reliable transportation, and current automobile insurance
- Strong assessment, clinical judgment, and critical thinking skills
- Excellent communication and organizational abilities
Qualifications
- Preferred: Hospice experience, palliative care experience, home health experience, case management experience
- Experience working independently in the field

Pay
$80,000 – $90,000 annually
