Analyst, Case Mgt-Field
About the role
As a Case Manager Analyst you support optimal member health outcomes by coordinating care, providing education, and facilitating access to appropriate healthcare services. Using care management tools and data analysis, you assess member needs, identify risks, and develop and implement effective care plans. You collaborate with internal teams, providers, and external resources to address barriers, improve quality of care, and ensure services align with member benefits. Through communication, coaching, and motivational techniques, you promote member engagement, informed decision-making, and healthy behavior changes. You are responsible for monitoring care plan progress, documenting activities, and ensuring compliance with regulatory standards, company policies, and quality management guidelines.
Responsibilities
- Coordinate care and provide education to support member health outcomes.
- Assess member needs and identify risks using care management tools and data analysis.
- Develop and implement effective care plans.
- Collaborate with internal teams, providers, and external resources to address barriers and improve quality of care.
- Promote member engagement, informed decision-making, and healthy behavior changes through communication, coaching, and motivational techniques.
- Monitor care plan progress and document activities.
- Ensure compliance with regulatory standards, company policies, and quality management guidelines.
Requirements
- Bilingual (English & Spanish): Required. Must successfully pass both verbal and written language assessments.
- Must have reliable transportation and be willing/able to travel up to 75% of the time. Mileage is reimbursed per company policy.
- Must reside in Miami-Dade County, FL within zip codes 33175, 33185, 33186 & 33193.
- At least 1 year of experience in a behavioral health or long-term care setting.
- Proficiency in Microsoft Office Suite and general computer use.
Qualifications
- Bachelor’s Degree or non-licensed Master’s level clinician required.
- Degree to be in behavioral health, human services (psychology, social work, marriage and family counseling).
Preferred Qualifications
- Experience in discharge planning, case management, or managed care (MCO).
- Strong ability to multitask, prioritize, and adapt in a fast-paced environment.
- Advanced communication skills in both English and Spanish (verbal and written).
Schedule
Full time, 40 anticipated weekly hours.
Pay
The typical pay range for this role is $21.10 - $40.90 per hour. This pay range represents the base hourly rate for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography, and other relevant factors. This position is also eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay range listed above.
