About the role
The Ambulatory Staff Pharmacist provides integrated support to patients and physicians through a 'hands-on' approach to total quality patient care, delivering a full continuum of medication management services. This includes preparing medications, providing pharmacological information, monitoring patient drug therapies, and offering adherence support.
Responsibilities
- Prepares and dispenses medications by reviewing and interpreting physician orders; detecting therapeutic incompatibilities.
- Provides thorough patient consultations on disease state management and medication therapies.
- Determines clinical appropriateness for immunizations; counsels and provides immunizations as indicated.
- Performs medication therapy management, including reviewing and monitoring medication regimens, advising clinicians on evidence-based therapy, and documenting interventions.
- Completes pharmacy operational requirements by organizing and directing technicians' workflow; verifying preparation and labeling of pharmaceuticals; verifying prescriptions.
- Handles care coordination, identifies and resolves insurance-related issues, and recognizes situations where financial assistance can be applied.
- Understands and works in a manner to support 340B mission and compliance; possesses 340B knowledge sufficient to audit a claim and assist in external audits.
- Complies with state and federal drug laws as regulated by the state board of pharmacy, the drug enforcement administration, and local policies and procedures.
- Maintains records for controlled substances; removes outdated and damaged drugs from inventory; supervises support personnel.
- Makes referrals to other health professionals as necessary.
- Develops own knowledge by participating in education programs; trains pharmacy staff, students, interns, externs, residents, and health care professionals.
- Manages, organizes, and updates relevant data using database applications.
- Applies observations and recommendations to operational issues, increased productivity, quality, and customer-service standards.
- Identifies trends, resolves problems, recommends improvements, and implements change.
- Actively participates in process improvement initiatives.
- Protects organization’s value by keeping information confidential.
- Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
- Performs other duties as assigned.
Requirements
- Doctor of Pharmacy (PharmD) or Bachelor's degree in Pharmacy.
- Licensed Pharmacist in the State of Rhode Island.
- 5+ years of experience in a specialty or retail pharmacy setting; 8+ years preferred.
- Preferred advanced certification(s) such as: immunization, medication therapy management, diabetic educator, cardiovascular educator, 340B ACE.
- Ability to foster a cooperative work environment.
- Strong interpersonal and communication skills; ability to work effectively with branch departments.
- Knowledge of computerized information systems (knowledge of EPIC Willow is a plus).
- Knowledge of specialty pharmacy reimbursement: state, federal, and commercial.
- Ability to maintain record-keeping systems and procedures.
- Knowledge of management principles and practices.
- Ability to develop, plan, and implement short- and long-range goals.
- Analyzes information, administers medication accurately, exercises good judgment, and possesses strong verbal communication skills.
- Deep knowledge of pharmacology developments and FDA health regulations.
- Ability to manage processes, exhibit strong technical and clinical skills with a constant focus on legal compliance.
- Highly productive, quality-focused, detail-oriented, and computer proficient.
