Responsibilities
- Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers.
- Facilitates flow through the system and facilitates interactions with the care team.
- Increase patient and provider satisfaction by eliminating barriers to timely care.
- Maintain point of contact with providers and serve as the first point of contact for patients and families.
- Work closely with pulmonary specialists to deliver comprehensive care to Asthma, COPD, and Bronchiectasis patient populations.
- Guide patients through their care journey.
- Coordinate the entire care delivery process for Asthma, COPD, and Bronchiectasis patients, starting from their initial referral.
- Arrange post-hospital visits and provide essential education.
- Promote adherence to treatment plans and connect high-risk patients with home care resources.
- Act as a liaison for the prior authorization process for specialty medications and necessary equipment.
- Develop and maintain a registry specifically for high-risk patients within the Center.
- Attend bi-monthly Airways Disease conferences to stay current and contribute to program development.
Requirements
- 3 years of experience, ambulatory experience preferred.
- The facility maintains a fragrance-free work environment.
Qualifications
- Must have a Bachelor's degree in Nursing or related field.
- Current New York State Registered Nurse License.
- Experience in ambulatory care setting preferred.
