Ambulatory Care Navigator - Population Health

About the role

Advocate Care Connect is a remote ambulatory nursing team that partners with Primary Care practices to coordinate care for patients with chronic and acute health conditions. The team works to address barriers to care, improve access to services, and support efforts to reduce avoidable healthcare utilization. We are seeking a dedicated and experienced Registered Nurse (RN) to join our team.

Schedule

Monday–Friday, 8 am to 5 pm; no weekends or holidays.

Pay

Pay range: $38.20 – $57.30 per hour.

Responsibilities

• Ongoing evaluation and quality improvement of the Navigator Program; track all activity as defined by departmental and program standards.
• Ensure quality patient care by adhering to WFBMC policies, involving patients and families in care planning, and communicating the plan across the continuum of care.
• Identify barriers to efficient and effective patient-care management and develop strategies to remove them.
• Implement outreach efforts to establish and maintain positive working relationships with key community agencies.
• Collect and maintain up-to-date, accurate program data and documentation according to program standards; deliver reports demonstrating outcomes and performance improvement activities.
• Adhere to established productivity benchmarks.
• Provide chronic care management using motivational interviewing and health coaching.
• Function in a professional practice model through active participation in shared governance and personal responsibility for professional growth.
• Participate in continuing education and apply constructive feedback to improve performance.
• Provide age- and developmentally-appropriate patient care in accordance with Age-Specific guidelines.
• Serve as a liaison between provider and patient between clinic visits; communicate by phone (or in person if needed) when clinical concerns arise and provide transitional care after ED or hospital visits.
• Identify and address barriers to care and assist with community-resource identification.
• Meet weekly with the primary medical team to discuss current patients and identify new at-risk patients.
• Participate in Transitional and Supportive Care rounds to identify service candidates.

Requirements

• Graduation from an accredited School of Nursing.
• Current license as a Registered Nurse (RN) in the applicable state.

Qualifications

• BSN preferred.
• Two years’ experience as a Registered Nurse.
• Case management experience preferred.
• Bilingual (Spanish) preferred.
• Problem-solving, decision-making, and critical-thinking skills.
• Ability to work in a self-directed environment and lead teams.
• Ability to work in a fast-paced environment with multiple interruptions.
• Expertise in MS Office and ability to prepare program reports efficiently.
