For our client, we are seeking a Ambulatory Care Clinical Pharmacist - Massachusetts 100% REMOTE (MA licensed required)) to join the team of a leader in the Healthcare space. This role will support ambulatory care patients through comprehensive medication management in a fully remote setting. The pharmacist will work closely with clinical care teams and external providers to develop medication plans, align interventions, and improve outcomes for members with complex medication needs. This position also contributes to staff education, clinical pathway development, and the ongoing refinement of medication management processes. Location: Remote - US based candidates only, no visa sponsorship available Compensation: $150,000 – $162,000 annually Responsibilities Provide Comprehensive Medication Management (CMM) in collaboration with clinical care teams. Develop medication management action plans with measurable goals and timelines. Communicate with care teams to align clinical interventions with medication goals. Support clinical team members with drug information and member-specific needs. Conduct staff education on medication management and care management platforms. Help create clinical pathways and protocols for medication management. Qualifications Active Massachusetts pharmacist license required and must remain in good standing. Completion of PGY-1 and PGY-2 residencies, or 2+ years as a clinical pharmacist in ambulatory care. Strong understanding of Comprehensive Medication Management (CMM) principles. Experience collaborating with healthcare teams and external providers. Ability to develop and implement medication management plans.
