All Pointe HomeCare LPN - Per Diem - Various Locations, CT
Responsibilities
- Aid in preparing, implementing, and evaluating the client's care plan.
- Monitor and document client symptoms, reactions, and changes, including physical and mental conditions, and report any concerning signs or symptoms.
- Create clinical and progress notes for review by the clinical team.
- Instruct clients on appropriate self-care techniques and daily living skills.
- Foster collaboration among healthcare professionals through participation in team meetings and activities.
- Pursue personal and professional development through involvement in nursing organizations, in-service programs, and on-the-job learning opportunities.
Qualifications
- Current LPN license in the state of Connecticut.
- Experience in home care or pediatric settings is preferred.
- Strong knowledge of medical terminology and diagnostic evaluation processes.
- Excellent communication skills to interact effectively with children and their families.
