Responsibilities
- Oversee the implementation of health-related services for the Assisted Living Residence (ALR) and Assisted Living Program (ALP).
- Participate in the assessment and screening of applicants to determine appropriateness for care.
- Aid in the development and implementation of individualized service plans (ISP) based on assessments.
- Conduct assessments and reassessments to identify residents' healthcare needs and adjust care plans as necessary.
- Document resident care and changes in status.
- Supervise program nurses and home health aides.
Qualifications
- Graduate of an accredited school of Nursing (BSN).
- Current Registered Nurse New York State License.
- Experience in staff supervision and medication oversight in Assisted Living, Nursing Homes, or similar environments.
- Strong management and supervisory skills, with experience in assessments, care planning, and reassessments.
- Experience with Dementia and/or Alzheimer’s patients is a must.
- Minimum of 2 years of experience providing services to older adults in settings such as home care, residential care, adult care facilities, or rehabilitation.
Pay
$60.0961 Hourly
