About the role
The Afterhours Care Manager RN provides high-quality, compassionate hospice care outside standard business hours. Key responsibilities include:
- Assessing and coordinating patient and family needs during after-hours shifts.
- Maintaining primary clinical oversight for assigned patients.
- Working as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team.
- Developing and revising the Hospice Plan of Care in collaboration with physicians and IDG members.
- Educating patients and families on disease progression, symptom management, medications, and hospice philosophy.
- Communicating significant changes in patient condition promptly to hospice and attending physicians.
- Maintaining accurate, timely clinical documentation including assessments, care plans, and notes.
- Supporting administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection.
- Ensuring infection control standards and universal precautions are consistently followed.
- Providing leadership and guidance to field staff; conducting supervisory visits when appropriate.
- Participating in required meetings, after-hours updates, and trainings.
- Promoting the organization's core values and maintaining a culture of compassion, professionalism, and teamwork.
- Performing other duties as assigned.
Responsibilities
The Afterhours Care Manager RN is responsible for:
- Ensuring appropriate delivery of hospice services to eligible patients and families.
- Providing direct in-person patient care during on-call, weekend, and after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges.
- Conducting initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs.
- Serving as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team.
- Developing and revising the Hospice Plan of Care in collaboration with physicians and IDG members.
- Supporting the IDG process through chart reviews, plan of care updates, and interdisciplinary communication.
- Communicating significant changes in patient condition promptly to hospice and attending physicians.
- Maintaining accurate, timely clinical documentation including assessments, care plans, and notes.
- Assisting with rescheduled, missed, or reassigned visit requests based on triage direction.
- Participating in calendar and scheduling modifications, including service codes and clinician assignments.
- Supporting administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection.
- Ensuring infection control standards and universal precautions are consistently followed.
- Providing leadership and guidance to field staff; conducting supervisory visits when appropriate.
- Participating in required meetings, after-hours updates, and trainings.
- Promoting the organization's core values and maintaining a culture of compassion, professionalism, and teamwork.
- Performing other duties as assigned.
Requirements
Requirements for the Afterhours Care Manager RN include:
- A current RN license in the state of employment.
- A graduate of an accredited nursing program.
- At least one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
- Meeting any state-specific requirements.
- An active RN license.
- CPR certification (in-person).
- A valid driver’s license, reliable transportation, and current auto insurance.
Skills and Competencies

- A strong knowledge of hospice principles, Medicare/Medicaid regulations, and best practices.
- Effective communication skills with patients, families, physicians, and interdisciplinary team members.
- Strong time-management and prioritization skills in a fast-changing environment.
- Proficiency with EMR systems (HomeCare HomeBase preferred), Microsoft Office, and mobile technologies.
- Leadership skills to support and guide field staff.
- Ability to manage administrative, documentation, and compliance responsibilities accurately.
