Responsibilities

• Support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care.
• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.
• Maintain accurate documentation and reports.
• Coordinate community and educational services for families.
• Meet with families in-person to support reunification and resolve any concerns.

Requirements

• Bachelor’s degree in human services.
• Valid driver’s license.
• Willingness to learn—training will be provided.

• Advocate for the families and children served.
• Eager to learn and grow in skills to better support families and children.

Pay

Average hourly rate is $19.23 (including wage, incentives, bonuses, overtime, shift differential, etc.).
