About the role
We're seeking a compassionate Licensed Practical Nurse (LPN) to join our hospice After Hours team for a PRN opening. As an LPN, you'll provide direct patient care under the supervision of a Registered Nurse, following individualized care plans and ensuring every patient receives the highest standard of comfort and support. You'll collaborate closely with an interdisciplinary team to deliver care that meets physical, emotional, and spiritual needs.
Responsibilities
- Provide direct patient care as outlined in the plan of care established by the Registered Nurse.
- Implement current nursing practices based on comprehensive assessments and individualized care plans.
- Document care accurately and in a timely manner in the EMR—right at the bedside—to reflect the plan of care.
- Educate and support patients, families, and caregivers regarding diagnoses and self-care plans as directed by the RN or provider.
- Manage medical equipment and supplies effectively and efficiently to ensure seamless patient care.
- Provide appropriate pain and symptom management, evaluate patient responses to treatments and medications, and collaborate closely with the RN and provider regarding changes or concerns.
- Facilitate cohesive, team-based patient care in alignment with organizational quality initiatives, policies, and protocols.
- Participate in team meetings, interdisciplinary collaboration, quality improvement initiatives, and ongoing professional growth and development.
- Maintain reliable transportation, including a valid driver’s license and insured vehicle, and be willing to travel throughout the full service area.
- Perform additional duties as assigned by the manager or team leader to support exceptional patient care.
Requirements
- Graduate of an accredited practical or vocational nursing program.
- Current LPN license in Virginia.
- CPR certification and at least two years of nursing experience (community health, hospice, or medical-surgical preferred).

- Reliable transportation, valid driver’s license, and automobile insurance.
