After-Hours Admissions Nurse
Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education. Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life-threatening diagnosis through death and bereavement.
About the role
The ideal candidate will provide coverage for After-Hours to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.
Responsibilities
- Make in-person nursing visits to patients as assigned, or as indicated.
- Provide continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations.
- Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals.
- Provide effective management of symptoms, including prompt escalation to providers or others as indicated.
- Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services.
Requirements
- Associate's or Bachelor’s degree in nursing from an accredited School of Nursing.
- Current license as a registered nurse in the State of California.
- Public Health Nursing certification highly desirable.
- HPNA certification preferred.
Qualifications
- Experience in acute care, home health, and/or hospice setting preferred.
Schedule
Monday - Friday, 4:00pm - 12:30am.
Location
Alameda County with additional coverage to San Mateo County as needed.
Pay
$55.50 - $69.50 per hour, plus after-hours/weekend shift differential.
