Primary Responsibilities
- Initiate, organize, plan, develop, and implement direct advocacy services to assigned participants and their families.
- Develop and implement service plans based on a strength-based approach using the wrap around model.
Qualifications/Requirements
- Minimum High School Diploma or GED.
- Experience in community work and knowledge of community resources.
- Experience working with at-risk youth.
- Excellent verbal and written communication skills.
- Proficient in Microsoft office suite; familiarity using an electronic health record system is a plus.
- CPR/First Aid Certification is a plus.
- Bilingual (Spanish speaking) is a plus.
- Reliable insured transportation, valid driver's license, and current auto insurance coverage is required.
