About the Role

We are currently seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide in-home assessments, care coordination, and clinical support for Medicare Advantage members throughout Wake County, North Carolina.

Responsibilities

  • Conduct comprehensive in-home health assessments for Medicare Advantage members
  • Perform medication reviews, health screenings, and risk assessments
  • Identify medical, behavioral, and social factors that may impact patient outcomes
  • Educate patients and caregivers on disease management, medications, and preventive care
  • Develop and support individualized care plans based on patient needs
  • Collaborate with primary care providers, specialists, and ancillary healthcare partners
  • Coordinate care
  • Identify care gaps and recommend interventions to improve quality outcomes
  • Document patient encounters and care recommendations accurately within the electronic medical record
  • Support initiatives focused on reducing hospitalizations, improving chronic disease management, and enhancing patient quality of life

Qualifications

  • Master's Degree from an accredited Nurse Practitioner or Physician Assistant program
  • Active North Carolina Nurse Practitioner or Physician Assistant license
  • Active board certification as a Nurse Practitioner or Physician Assistant
  • Active NPI Number
  • Active DEA Registration
  • Current BLS Certification
  • Valid driver's license and reliable transportation with current automobile insurance
  • Strong communication and relationship-building skills
  • Excellent organizational, problem-solving, and time management abilities
  • Ability to work independently in a fast-paced field-based environment

Preferred Skills

  • One or more years of clinical experience as an NP or PA
  • Home health, home-based primary care, or community-based care experience
  • Experience caring for geriatric or Medicare populations
  • Chronic disease management experience
  • Previous electronic medical record (EMR) experience
  • Experience working within value-based care or managed care environments
