Palm Medical Centers is a well-established provider of value-based primary care services. We are seeking to hire a Primary Care Nurse Practitioner to join our premier team.

Responsibilities

• Manage chronic disease, preventative care, and quality metrics in a value-based care model
• Document and build excellent relationships with patients
• Use EMR systems (e.g., eCW) proficiently

Requirements

• At least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Active and unrestrictive Florida APRN license
• DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (e.g., eCW a plus!)
