Responsibilities

  • Deliver personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities.
  • Provide dementia-informed, relationship-based care across the continuum of care.
  • Conduct comprehensive assessments and develop tailored care plans for older adults.
  • Manage chronic conditions with skill and empathy for patients and their families.
  • Collaborate with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care.
  • Build trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.
  • Maintain consistency in documentation, follow-through, and patient communication.

Requirements

  • Active, unencumbered NP or PA license in the state you will be practicing.
  • Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
  • Compassionate, patient-centered approach and strong clinical judgment.
  • Valid driver’s license and reliable transportation.

Pay

Pay range for posted region: $70,000 USD - $110,000 USD
