Responsibilities
- Deliver personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities.
- Provide dementia-informed, relationship-based care across the continuum of care.
- Conduct comprehensive assessments and develop tailored care plans for older adults.
- Manage chronic conditions with skill and empathy for patients and their families.
- Collaborate with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care.
- Build trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.
- Maintain consistency in documentation, follow-through, and patient communication.
Requirements
- Active, unencumbered NP or PA license in the state you will be practicing.
- Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
- Compassionate, patient-centered approach and strong clinical judgment.
- Valid driver’s license and reliable transportation.

Pay
Pay range for posted region: $70,000 USD - $110,000 USD
