About the role
The Bridge Clinic Advanced Practice Provider will play a critical role in an innovative care team dedicated to addressing health disparities in the community. This provider will support initiatives to improve access and provide comprehensive care to marginalized individuals, focusing on those experiencing homelessness.
Responsibilities
- Provide direct patient care in a clinic setting, mobile setting, and respite care setting.
- Collaborate with community partners, case managers, community health workers, and other healthcare professionals to support individuals in the Bridge Program.
- Collaborate with the Bridge Clinic Physician Lead to provide care within the bridge team.
- Participate and lead quality and care review meetings for patients in the Bridge Program.
- Communicate effectively with patients, families, colleagues, nursing, and other healthcare professionals.
- Prepare and provide necessary, timely, and accurate reports and forms as required by Central Health or the facility.
- Perform medical and administrative services under general guidance and minimal supervision, with accountability for specific organizational-level goals.
- Work closely with families of diverse patient populations.
- Help develop and execute plans for service delivery and patient experience at mobile sites.
- Prioritize duties and responsibilities, demonstrating strong organization and time management skills.
- Show excellent verbal and written communication skills, ensuring appropriate confidentiality is maintained.
- Interact positively and professionally with others, contributing to a positive team environment.
Requirements
- Holds and maintains an unrestricted license to practice as a Physician Assistant in the State of Texas OR an unrestricted license to practice nursing in the State of Texas and current credentialing as an Advanced Nurse Practitioner by the Texas Board of Nurse Examiners.
- Holds a current Drug Enforcement Agency (DEA) certification for writing prescriptions.
- Holds a current Healthcare Provider Cardiopulmonary Resuscitation (CPR) through the American Heart Association.
- Holds a current Basic Life Support Certification for Healthcare Providers through the American Heart Association.
Qualifications
- Graduation from an accredited school of physician assistants OR a master’s degree from an accredited school of nursing.
