Position Summary
Blue Zones Health Centers seek a compassionate and experienced Advanced Practice Provider (APP) to provide high-quality outpatient primary care for an established patient panel in our Riverside, CA clinic. The ideal candidate is passionate about preventive medicine, chronic disease management, patient education, and empowering patients to make sustainable lifestyle changes.
Key Responsibilities
- Provide comprehensive primary care services for an assigned panel of adult patients in an outpatient clinical setting through both in-person and telehealth (video and audio) appointments.
- Diagnose, treat, and manage acute and chronic medical conditions using evidence-based clinical practice guidelines.
- Apply Lifestyle Medicine principles including nutrition, physical activity, restorative sleep, stress management, social connection, and avoidance of risky substances to prevent, treat, and help reverse chronic disease whenever appropriate.
- Perform comprehensive patient assessments, develop individualized treatment plans, prescribe medications, order diagnostic testing, and coordinate follow-up care.
- Educate and motivate patients to improve long-term health through sustainable lifestyle modifications and preventive care strategies.
- Collaborate with physicians, specialists, behavioral health providers, care managers, community organizations, and interdisciplinary teams to ensure coordinated, high-quality patient care.
- Manage electronic health record (EHR) documentation, patient communications, medication refills, test results, and provider inbox responsibilities in a timely manner.
- Participate in quality improvement initiatives, population health programs, value-based care metrics, and patient outcome improvement efforts.
- Provide rotating on-call coverage according to clinic schedule.
- Maintain accurate clinical documentation while ensuring compliance with federal, state, payer, HIPAA, and organizational standards.
- Contribute to a collaborative, patient-centered culture focused on continuous improvement and exceptional care delivery.
- Comply with all applicable federal, state, and local laws, as well as all Employer policies, procedures, and standards, including but not limited to codes of conduct and ethics requirements, as amended from time to time.
Requirements
- Required: Current, active, and unrestricted U.S. license as a Nurse Practitioner (FNP, AGNP, or ANP) or Physician Assistant (PA-C).
- Required: National board certification in good standing.
- Required: Minimum of 2 years of clinical experience in primary care, family medicine, internal medicine, adult medicine, or ambulatory care.
- Required: Strong clinical assessment, diagnostic reasoning, and evidence-based decision-making skills.
- Required: Experience managing patients with chronic conditions such as diabetes, hypertension, obesity, cardiovascular disease, and metabolic disorders.
- Required: Proficiency with electronic health records (EHR/EMR), telemedicine platforms, and clinical documentation.
- Required: Excellent communication, patient education, and motivational interviewing skills.
- Required: Ability to work independently while collaborating effectively within a multidisciplinary healthcare team.
- Required: Strong organizational, time management, and problem-solving abilities.
- Preferred: Certification from the American College of Lifestyle Medicine (ACLM).
- Preferred: Conversational Spanish.
- Preferred: Experience in value-based care, population health, accountable care organizations (ACO), or risk-based healthcare models.
- Preferred: Experience with care coordination, quality metrics, and interdisciplinary team-based care.
