Responsibilities
- Expected average patient load of 2 visits per hour
- Collaborative environment with onsite lab, behavioral health therapists, and nutritionists
- Innovative, integrated use of telehealth
- Centralized care coordination, care management, and population health management
Requirements
- Nurse Practitioner or Physician Assistant with an active North Carolina State License
- Located in Cary, NC

- 2 or more years of experience
- Strong desire to become a partner with Avance Care
- Commitment to practice mindful medicine and treat patients like people rather than numbers
