About the role

The Advanced Practice Provider-Full-Time Intellectual And Development Disabilities (IDD) Services role at SummitStone Health Partners is dedicated to providing integrated care for individuals with complex behavioral health, substance use, and medical needs, particularly those involved in the criminal justice system and experiencing homelessness or housing instability. This role supports individuals with intellectual and developmental disabilities through collaborative care with multidisciplinary teams, community partners, and external systems.

Responsibilities

  • Evaluate, diagnose, and treat individuals with mental health, substance use, and co-occurring medical conditions
  • Provide psychiatric evaluation, medication management, and primary care services within scope of practice
  • Perform physical assessments, review medical histories, interpret clinical data, and complete procedures such as injections, wound care, and other minor medical interventions
  • Manage acute and chronic conditions, including crisis intervention and emergency stabilization for withdrawal, intoxication, or other acute medical conditions
  • Participate as the medical representative within multidisciplinary treatment teams to support integrated care
  • Develop, implement, and monitor individualized, person-centered treatment plans
  • Educate and counsel individuals on treatment plans, medication adherence, and overall health and wellness
  • Care for and coordinate care and support continuity across levels of care, providers, and service systems
  • Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory standards and adhere to all organizational policies, clinical standards, and applicable state and federal regulations
  • Participate in on-call responsibilities, travel between service locations and partner organizations, and work across inpatient, outpatient, residential, and community-based settings as needed to support service delivery and organizational operations

Requirements

  • Master’s degree from an accredited program in Advanced Practice Nursing (APN) or Physician Assistant (PA) studies required
  • Four (4) years of experience in a related clinical field required
  • For Physician Assistants (PA): Master's degree in physician assistant studies, Active, unrestricted license to practice as a Physician Assistant in the State of Colorado, National certification as a Physician Assistant (PA-C) issued by the National Commission on Certification of Physician Assistants (NCCPA), maintained throughout employment
  • For Nurse Practitioners (NP): Master's degree in nursing from an accredited Nurse Practitioner program, Active, unrestricted Nurse Practitioner license in the State of Colorado (or compact privileges, if applicable), Active, unrestricted Registered Nurse (RN) license in the State of Colorado, or ability to obtain within 30 days of hire, National certification as a Nurse Practitioner, including Family Nurse Practitioner (FNP) or Psychiatric Mental Health Nurse Practitioner (PMHNP), issued by ANCC or AANPCB and maintained throughout employment, Full or provisional prescriptive authority eligibility; must obtain prescriptive authority as a condition of employment, Prescriptive authority in accordance with state regulations and organizational privileging processes

Qualifications

  • Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross, maintained throughout employment
  • Valid Colorado driver’s license (or ability to obtain within 30 days of hire) and a clean driving record
  • Valid Colorado driver's license if position responsibilities require travel between community locations

  • Understanding of unique healthcare challenges faced by individuals with intellectual and developmental disabilities
  • Commitment to promoting dignity, inclusion, access to care, and improved health outcomes
  • Knowledge of person-centered planning principles
  • Experience collaborating with community agencies and support systems
  • Spanish language skills or other bilingual capabilities
