Advanced Practice Provider (Hybrid)

Key Responsibilities

• Conduct monthly in-person visits to assigned healthcare facilities.
• Assigned to manage between 1 - 10 Tapestry affiliated facilities.
• Deliver and review patient CCM, RPM, Daily Risk reports and BHI reports with facility staff.
• Perform Initial Preventive Visits (IPV) for new patients and re-admits as required for enrollment of services.
• Facilitate discussions with medical professionals, ensuring alignment between data insights and patient care.
• Ensure clinical observations support report accuracy and provide valuable insights into patient health trends.
• Collaborate with facility staff to enhance proactive care planning and improve patient outcomes.

Qualifications

• Licensed nurse practitioner.
• Experience in chronic care management, remote patient monitoring, or behavioral health integration preferred.
• Strong interpersonal and communication skills for effective collaboration with medical staff.
• Able to travel to multiple healthcare facilities on a scheduled basis.
• Detail-oriented with the ability to analyze clinical reports and identify critical insights.

Pay

The anticipated annual salary for this role is $140K - $145K with benefits including 401k, PTO, and CME. Mileage reimbursement is provided for travel involved in the role. This hybrid position follows a location-based compensation structure.
