Advanced Practice Provider - Geriatrics
Responsibilities
Championing clinical directorship and comprehensive clinical services in a skilled nursing facility setting.
Delivering primary care, chronic disease management, and patient-centered care to adult and geriatric populations.
Playing a key role in daily patient management, regulatory compliance, timely assessments, and continuity of care.
Requirements
Seeking a Certified Registered Nurse Practitioner (CRNP) – Adult-Gerontology NP (AGNP) or Family Nurse Practitioner (FNP).
Monday–Friday schedule with flexible hours supporting strong work-life balance.
Shared call rotation with provider team: One weekday night per week.
Every sixth weekend.
Qualifications
Currently supports 110 WellSpan patients (short-term rehab & long-term care).
Join an established, collaborative care team: 2 Physicians 3 Nurse Practitioners.
Ideal role for NPs interested in long-term care, rehabilitation medicine, geriatric care, and complex adult medicine.
