Advanced Practice Provider - Geriatrics

Our Mobile Primary Care Team is seeking a dedicated and compassionate Physician Assistant or Nurse Practitioner with a heart for senior care and a passion for redefining how older adults and their caregivers experience health and well-being. This position blends independent practice and team-based collaboration, supported by a model that values compassion, connection, and clinical excellence. Help solve the rural area access-to-care crisis in Colorado — this role is based in Pueblo, with travel to Trinidad, Rocky Ford, and Fowler to bring high-quality, relationship-based care to communities that need it most.

Responsibilities

• Deliver personalized high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities.
• Travel across Pueblo, Trinidad, Rocky Ford, and Fowler to extend care access to rural and underserved communities.
• Deliver dementia-informed, relationship-based care across the continuum of care.
• Conduct comprehensive assessments and develop tailored care plans for older adults.
• Manage chronic conditions and perform care with skill and empathy for patients and their families.
• Collaborate with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care.
• Build trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.
• Maintain consistency in documentation, follow-through, and patient communication.

Requirements

• Active, unencumbered NP or PA license in the state you will be practicing.
• Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
• Compassionate, patient-centered approach and strong clinical judgment.
• Valid driver's license and reliable transportation, with willingness to travel to Pueblo, Trinidad, Rocky Ford, and Fowler.

Pay

Pay Range for Posted Region: $85,000 USD - $150,000 USD (productivity-based).
