Advanced Practice Provider
About the role
An advanced practice provider position is available for a PRN (per diem) schedule to assist with hospital or clinic coverage during weekdays or weekends. This role involves advanced nursing practice, including the care of assigned patient populations, analysis of health data, development and implementation of a care plan, comprehensive clinical coordination, case management, consultation, and education.
Responsibilities
- Collect and analyze client health data
- Identify appropriate expected outcomes
- Develop and implement a plan of care
- Provide comprehensive clinical coordination of care
- Perform case management
- Offer consultation to influence the plan of care for clients
- Enhance the abilities of others and effect change in the system
- Use prescriptive authority, procedures, and treatments in accordance with state and federal laws and regulations
- Evaluate clients' progress towards achieving expected outcomes
- Provide education to patients, their families, and nursing staff
- Manage high-risk patients
Requirements
- Applicable experience: 1 year
- Advanced Practice Provider - Various
- Master's Degree
