RWJBarnabas Health, Primary Care Service Live is seeking a Full-Time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to be part of the primary care service line providing coverage to southern NJ.

Qualifications

• CDS and DEABLS
• Master's degree required or Graduate Level Certification
• APNs
• Current NJ APP
• RN License
• PAs
• PA license
• NCCPA

Essential Functions

• Performs comprehensive initial and episodic health assessments
• Initiates and implements an individualized nursing and medical plan of care in collaboration with members of interdisciplinary team
• Initiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physician
• Performs follow-up care including acute and routine examinations
• Maintains and adjusts patients medication according to protocol or medical plan of care
• Implements nutritional therapies, as appropriate
• Triage patient problem calls which requires medical attention
• Orders appropriate laboratory and diagnostic procedures
• Recognizes normal and abnormal laboratory findings and the pathology involved
• Documents information in patients charts following documentation standards
• Provides health education to the patient and family
• Manages medical emergencies as indicated, and according to standing orders
• Monitors and adjusts patients medication according to protocol or medical plan of care
• Implements nutritional therapies, as appropriate
