Essential Functions
- Performs comprehensive initial and episodic health assessments;
- Initiates and implements an individualized nursing and medical plan of care in collaboration with members of interdisciplinary team;
- Initiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physician;
- Performs follow-up care including acute and routine examinations;
- Makes appropriate adjustments to patients' medication according to protocol or medical plan of care;
- Implements nutritional therapies, as appropriate;
- Triage patient problem calls which require medical attention;
- Orders appropriate laboratory and diagnostic procedures;
- Recognizes normal and abnormal laboratory findings and the pathology involved;
- Documents information in patients' charts following documentation standards;
- Provides health education to the patient and family (e.g., management of side effects, anticipatory teaching);
- Manages medical emergencies as indicated, and according to standing orders;
- Monitors and adjusts patients' medication according to protocol or medical plan of care;
- Implements nutritional therapies, as appropriate;
