Job Overview

RWJBarnabas Health, Primary Care Service Live is seeking a Full-Time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to be part of the primary care service line providing coverage to northern NJ.

Qualifications

  • CDS and DEABLS
  • Master's degree required or Graduate Level Certification
  • APNs
  • Nurse Practitioner Certification
  • RN License
  • PAs
  • PA license
  • NCCPA

Essential Functions

  • Performs comprehensive initial and episodic health assessments;
  • Initiates and implements an individualized nursing and medical plan of care in collaboration with members of interdisciplinary team;
  • Initiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physician;
  • Performs follow-up care including acute and routine examinations;
  • Makes appropriate adjustments to patients' medication according to protocol or medical plan of care;
  • Implements nutritional therapies, as appropriate;
  • Triage patient problem calls which require medical attention;
  • Orders appropriate laboratory and diagnostic procedures;
  • Recognizes normal and abnormal laboratory findings and the pathology involved;
  • Documents information in patients' charts following documentation standards;
  • Provides health education to the patient and family (e.g., management of side effects, anticipatory teaching);
  • Manages medical emergencies as indicated, and according to standing orders;
  • Monitors and adjusts patients' medication according to protocol or medical plan of care;
  • Implements nutritional therapies, as appropriate;
  • Triage patient problem calls which require medical attention;
  • Orders appropriate laboratory and diagnostic procedures;
  • Recognizes normal and abnormal laboratory findings and the pathology involved;
